So here is an up close view of the general report sheet format I use. I am considering recreating the digital format again and may post that file later. I am also going to try and post the accompanying video from my IG story so that you can see the explanations of everything. Later I'll add my tips for a stupendous change of shift report!

• This report sheet is meant to be printed on both sided of one piece of paper and folded in half to provide you with info for 4 patients. 
• Please be patient with my limited initial draft of this. I personally prefer to draw it each time I go to work.
